Senior Class Parents/ Guardians

NAME ON DIPLOMA AND SENIOR CLASS PICTURES

It is time to begin thinking about how the seniors would like their names to appear on their diplomas. We would like you to take a moment and discuss with your senior(s) a choice of name that would be agreeable to both you and your senior(s). We are requesting the use of the middle name due to the high occurrences of duplication and the need for verification of names at the college level.

Examples include: First Name, Middle Name, Last Name First Name, Middle Initial, Last Name

After deciding how the name should appear on the diploma, please clearly print it on the line below.

The other matter that needs attention is pictures for the class composite and the newspaper. Each senior needs to bring two (If pictures were taken at Studio B) or three billfold size graduation pictures to the senior class sponsors as soon he or she receives them from the photographer. The administration has requested that no caps or hats are worn in any of these pictures.

Please sign and return this form to Mrs. Alder or Mrs. Larabee by October 31, 2003.
