Please have your medical reimbursement sheets to me by 4:00 p.m., Tuesday, December 7, 2004. I have a link that you can click on and print the form you need for this. Reimbursement checks will be issued on December 10, 2004.
Eligible Expenses include the following:
Ambulance service Birth control contraceptives Contact lens solutionCrutchesDrugs(legal, prescription only or insulin)Eye glasses/contact lensesMedical copayments and deductiblesNeedles, syringes and other diabetic-related suppliesMedically prescribed therapy treatmentsOther various medical care expenses listed in your book <?font>
